Vaccine Requirement Exemption Forms

Individuals requesting an exemption from the UNM COVID-19 vaccination requirement should complete only one of the appropriate forms below. Completed forms should be submitted via the online Vaccine Verification website and will be routed to the appropriate UNM office for review and approval.

Disability/Medical Condition/Pregnancy/Lactation Status Accommodation Request Form

Individuals at the Health Sciences Center with pending or approved accommodation or exemption requests are subject to regular COVID testing and must provide proof of a negative COVID-19 PCR test every 7 calendar days. COVID-19 testing will be at the expense of the employee or student seeking the exemption; the state of New Mexico has contracted with a test provider that currently will provide tests free of charge and this service may be used.

Note that effective Monday, May 16th weekly COVID testing is no longer required for main and branch campus faculty, staff, and students.

Utilize this form if:
  • You are an employee or student with a disability or medical condition that prevents you from receiving the COVID-19 vaccine
  • You are an employee or student who is currently pregnant and experiencing a high-risk pregnancy
  • You are an employee or student who is requesting an exemption for the COVID-19 vaccine and are either nursing or are currently pregnant and experiencing a typical pregnancy

Religious Accommodation Request Form

  • You are an employee or student who holds a sincere religious belief that prevents you from receiving the COVID-19 vaccine

Staff Remote Work Exemption Request Form

Utilize this form if:

  • You are a staff member that is working completely remotely and are requesting an exemption on that basis.

Note: Due to the requirements of the current State of NM Public Health Order, HSC employees are not eligible for the remote work exemption from the vaccine requirement.

Faculty Remote Work Exemption Request Form

Utilize this form if:

  • You are a Main or Branch Campus faculty member that is working completely remotely and are requesting an exemption on that basis.
  • Faculty members seeking approval for the remote working vaccine mandate exemption must meet the following criteria:
    • Previously approved remote work arrangement agreement between a faculty member and their department chair;
    • Live outside of New Mexico, and;
    • Scheduled to teach exclusively online classes for the semester in which the exemption is requested. All courses taught by the instructor must appear in the schedule as “Online.”
  • Department chairs and deans, by approving a request, certify that the faculty member is working entirely off-campus, meets the eligibility, and will not come on to campus for any reason.

 

Remote Online Student Exemption Form

Utilize this form if:
  • You are a student who is enrolled in fully online courses and will provide verification from an academic advisor. You will also attest that you will not come on campus for any purpose during the semester.

  • Students who have been granted a limited remote online exemption from the UNM Administrative Mandate on Required Vaccination should be aware that not all degree programs at UNM can be completed entirely online. For a list of degrees that can be completed entirely online, see https://online.unm.edu/online-degrees/index.html. A limited remote online exemption exempts a student from providing proof of vaccination based on the student's commitment not to visit UNM's physical campuses during the semester of exemption. The limited remote online exemption only pertains to the UNM Administrative Mandate on Required Vaccination. It does not apply to or guarantee enrollment in online classes, availability of classes online, or availability of degree programs online. Students who have been granted a limited remote online exemption should work closely with their advisors to develop a feasible plan for degree completion.

 

Temporary EUA Exception

On Monday, August 23rd the FDA granted full approval to the Pfizer-BioNTech coronavirus vaccine for people 16 and over. With this approval and pursuant to UNM's administrative mandate requiring COVID-19 vaccination, the Temporary EUA Exception is no longer permitted.
